Job Description – Community Support Worker ROLE SUMMARY A Community Support Worker enables Service Users with learning disabilities to live a full and rewarding life within their own home/community The key objective of the role is to provide the level of support required by each individual service user to fill the gap between what they are personally able to do and what they actually need to be able to do to lead an independent life All job-holders are required to work within the framework of the individual service users care plans and shared action plans to ensure that all of their needs are met Additionally, job-holders are required to assume a “Key Worker” role for a specific service user; assuming overall responsibility for their well being and acting as a vital link between the service user and all the other component parts of their life Specific Areas of Responsibility Managing Activities Knows and adheres to all Company policies and procedures Knows and adheres to the Company’s Philosophy of Care and Equal Opportunities Adheres to all legal/contractual requirements Enables service users to maintain and improve their mental health and well being Enables service users to maintain, develop and utilise a range of external relationships. Uses Resources Works to ensure the security and accuracy of all financial resources Works to maintain all physical resources and assets Works with People Promotes morale, motivation and team working Achieves high standards of performance and conduct Contributes to the training and development of oneself and the team Uses Information Contributes to the accuracy, availability, communication and confidentiality of all information
