Bridgwater Rehabilitation Team are looking for a Band 6 Physiotherapist to join their team.

The team works within the specialist Home First Reablement Unit based in Bridgwater Community Hospital. Patients are admitted to the unit for intensive reablement and are helped to achieve their goals as well as supported in returning to their home environment. The reablement team includes a dedicated nursing team, activity coordinator, physiotherapists, occupational therapists, reablement assistants and adult social care members.

Outside of the reablement unit Bridgwater rehabilitation team also work with Home First (pathway 1), and alongside the rapid response team as well as provide high quality rehabilitation to patients in the community to help people improve their quality of life and achieve what is important to them.

The team see adults with a range of conditions including those with neurological, orthopaedic, vascular, long term complex multi pathologies, frailty. The team also has a significant role in falls prevention work.

The rehabilitation team provides a 7 day per week service and the post holder will work 1 weekend day every month and will receive a day off in the week plus enhancements for weekend working.
