An exciting opportunity has come to work in the Neuro-rehabilitation care group as a Discharge Coordinator /Assessment Nurse working across two sites, Queen Mary’s Hospital, Roehampton and St. George's University Hospitals NHS Foundation Trust. We are looking for an experienced, motivated Staff Nurse preferably with experience in stroke or neurological rehabilitation to join our established Multidisciplinary team. The role will involve coordinating and planning admissions and discharges. Clinical assessment of patients referred to the service for rehabilitation. You will need to be an organized individual who is able to liaise with the multi-disciplinary team, patients & family members to co-ordinate and plan safe discharges for patients following Neuro-rehabilitation. In conjunction with the multi-disciplinary team, you will support the ward team in ensuring our patients are discharged in a safe and timely manner. This is a varied and challenging role which encourages you to use a problem solving approach and to be proactive.

You will have the opportunity to develop your teaching skills and participate in Trust wide initiatives to improve the discharge planning process.
