Our Care Coordinators are part of a team of professionals working with our NHS partners across Evesham, Bredon, Broadway & Inkberrow GP Practices. Our Care Coordinators provide extra time, capacity and expertise to ensure appropriate support is made available to all patients and make sure their needs are addressed.
Our Care Coordinators work directly with patients to develop holistic personalised care and support plans, giving patients more control over their own health and care. They encourage greater collaboration between GP's and their teams and community services, strengthen joint working with local partners, and help prevent illness and tackle health inequalities.
Who are we looking for?
We are looking for an excellent team player who is motivated, ambitious and a self-starter, with exceptional communication skills.
You will be confident within a multi-agency and disciplinary environment, and able to present and promote the Care Coordinator Service to a range of stakeholders.
Someone who wants to help develop this invaluable service and make a difference to tackling health inequalities, whilst playing an essential role within a growing a team of Care Coordinators across all the surgeries in the PCN.
