Service Support Worker - Assisted Discharge Service
Location: Bexhill (covering East Sussex)
Contract: Permanent
Hours per week: 18 hours (2-week rolling rota)
Salary: up to £18,838 (£9,688 for 18 hour week) per annum

We are looking for passionate and enthusiastic people with great interpersonal skills to join our Independent Living team in Bexhill-on-Sea.

The primary focus of the role is to support the provision of our Assisted Discharge Service for patients who are being discharged from either Eastbourne District General Hospital or the Conquest Hospital in East Sussex. Driving patients' home when discharged (using Red Cross vehicles) and making sure they are safely settled at home with everything they need. You will also support them by signposting and referring patients to other sources of support in the community. We want all service users to have a positive experience in the transition from hospital to home. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users' needs in order to re-enable service users to remain in their own home and prevent hospital re-admission.

You will respond to referrals from Clinicians and Community Health Professionals and will be required to have the ability to assess service users' needs, complete and follow a support plan for the individual. In addition, you will be providing practical and emotional support in the service user's own home and signposting to other services where appropriate to enable the individual to live independently in their own home.

Please note: You will also need a valid UK driving licence as this role requires travel within the East Sussex area using a Red Cross vehicle.
The position is for 2 shifts per week (including some weekends) the days to be confirmed. Shifts are 10am to 10pm on weekdays and 10am to 6pm at weekends.

* Educated to GCSE level (or equivalent by experience)

Frontline Workers COVID-19 statement This role requires use of full Personal Protective Equipment, including Fluid resistant face masks, to be worn at all times while delivering services. This will be provided for you. The role will involve frontline work with vulnerable services users and will not always allow for a 2-meter social distance to be maintained. You will be asked to complete a Risk assessment to confirm you are able to take up Frontline work, at onboarding stage. If you consider yourself to be a person at greater risk from Covid 19 or you are unable to wear a facemask for long periods of time then this role may not be suitable for you.
