A Support Worker is required by a non- profit organisation who provide Person Centred Support to vulnerable adults.

Job Description

Post- Support Worker

Responsible To- Team Leader

Summary of post - To provide, as a member of a staff team, high quality support and care to the individuals within the service.

Support Worker Responsible for:
  1. To implement and adhere to the organisation's philosophy.
  2. To understand and work within the parameters of all identified relevant legislation.
  3. To act as a keyworker to one or more individuals as required.
  4. Arrange and attend any review meetings providing up to date information in relation to the individual. This may include Person Centred Plans, monitoring forms and risk assessments. Ensure any information from meetings is documented fully and discussed with your line manager and colleagues
  5. Enable, assist, support and participate with the individual in all daily living tasks within their home, as identified with the individual's person-centred plan. This may include support with personal care and assessing and monitoring risks
  6. Give assistance and support to enable people to manage their own financial affairs. To monitor and keep good records with individuals
  7. To complete, monitor and review Personal Support Plans with the individuals we support as and when needed and at least twice a year. This will be done in consultation with Team Leader/Service Manager and other Stakeholders.
  8. Ensure that individuals have the correct amount of prescribed medication, by supporting and monitoring them in the management of their own medication.
  9. Empower and support each individual to take responsibility for his or her own decisions and actions ensuring that a positive approach is taken towards risk taking.
  10. To support all individuals using a framework of encouragement, enablement, positive motivation, and an understanding that every person has a right to be respected and valued.
  11. To be respectful and mindful when working in someone else's home.
  12. Develop and maintain good working relationships with Stakeholders.
  13. To work with all individuals in a professional, caring, supportive and courteous manner at all times.
  14. To work with all colleagues in a professional, caring, supportive and courteous manner at all times.
  15. Maintain up-to-date records, monitoring systems on a daily/weekly/monthly basis and provide written and/or verbal reports as requested. All documentation is to be completed to the standards set by the organisation which will also reflect those of regulatory bodies.
  16. Attend meetings, training courses, study days, seminars and other events as required.
  17. Attend and participate in any team meetings ensuring that any information regarding individuals that you work with or keywork is communicated to colleagues within this forum.
  18. Undertake on call and/or sleep in duties, on a rota basis, as required by the service.
  19. Preferably be a car driver and have access to a vehicle for the purpose of work, having a full licence and correct insurance cover or be able to access other means or transportation to enable travel to and from individuals' homes within the allocated travel time on a schedules.
  20. At the discretion of the organisation, to work within any team and to move when required to meet the needs of the service and the individuals we support.
  21. To adhere to the organisation's Policies and Procedures.
  22. To comply with all aspects of the Employee and Health and Safety handbooks.
  23. Carry out any other reasonable duties that are in the interest of the business.
Previous experience or qualifications aren't necessary- the organisation is happy to work with you to help you become an excellent Support Worker.

Work related Carer's parking permit, 4 week rolling rota- 2 in 4 weekends off, Long Service awards, Employee Assistance Programme, no experience required- full training given.

As of 1st April 2022, it will be a legal requirement for post holders in this role to be fully vaccinated against Covid-19, you will therefore be asked to provide evidence of vaccination status or medical exemption at interview.
