We are working with a number of LA 's that requires SEN Caseworkers.

The role will consist of coordinating the Education Health Care Needs assessment planning process for individual children and young people with special educational needs. You will act as a single point of contact for families throughout the assessment process and to work collaboratively with other agencies to secure the best possible outcomes for children and young people. You will also be working within statutory timescales and adhering to SEN legislation.

You will do:
To ensure the Education Health and Care (EHC) Plan assessment and maintenance is provided in a coordinated and personalised manner, which supports service users.
To coordinate EHC Person Centred Planning meetings with families and professionals across education, health, social care and voluntary sectors.
To liaise with a range of professionals to ensure their knowledge, experience, skills and relevant assessments inform the EHC plan.
To network with a variety of services, agencies and providers to gather information in order to support the family to make well informed choices, including parent support services.
To work collaboratively with partners to ensure the best overall outcome for the people of the county, whilst protecting their interests.

The roles are initially for 3 months and are full time.
