Spencer Clarke Group are looking for a Project Manager as part of the Long Term Conditions Hub within the NHS based in Derbyshire.
Contract - 3 month agency contract with a view to being extended on an ongoing basis.
Working hours - Monday - Friday 09:00 - 17:00
Rate of pay: £30+ per hour

About the role
We are looking for a highly committed and passionate individual who enjoys working in a fast-paced environment to join the Strategic Clinical Conditions and Pathways team as a Long-Term Conditions Hub Project Manager. The role will plan, co-ordinate, implement and monitor the Long-Term Conditions Hub pilot programme, supporting the Programme Lead to develop a hub model across three sites aimed to increase support to patients with LTCs.
Working with colleagues across the Derbyshire Integrated care system you will develop efficient and effective services aligned to local (STP) and national transformation. Taking a strategic approach, you will ensure that progress is measured and demonstrable to agreed timeframes. Therefore, first-rate communication skills and an ability to act with rigour and focus are essential.
About you
To be successful in this role you will have knowledge of long-term condition management and an understanding of service improvement and sustaining change techniques. Educated to post graduate degree level (or demonstrable equivalent level of experience).
Project Management qualifications e.g. PRINCE2;
Programme Management qualification such as MSP (Foundation level) or equivalent.
