We are working with an exceptional Neuro Rehab Organisation provider in the North East of England who are looking for experienced RGNs' or RMNs' who have the necessary skills and experience to care affectively to meet their patients' complex rehab goals. To work in collaboration with the clients, Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Psychologists, Case Managers, Support Staff and other Health and Social Care Professionals in client centred nursing care and rehabilitation.
To be aware of, implement and deliver Clients holistic care programmes. This individual programme is an integral part of each Clients 'Rehabilitation/ Care Plan'. It will be regularly monitored and reviewed to ensure continuity and consistency of the care and rehabilitation programme.
To motivate and support the client in achieving individual goals as determined through communication with the client, discussions with relatives/ advocates and input from other members of the health/ social care team via assessments, treatment and observation of the clients.
To facilitate, implement and assist the Clients care and rehabilitation programmes.
To act as a model to junior members of staff.
To complete designated tasks allocated by Management team.
To ensure nurse and carer records for each shift in charge are fully completed and correct.
To contribute towards clients ongoing assessment process.
To attend weekly and monthly MDT meetings and provide reports as directed.
To assist the Management Team in identifying Personal Assistant training needs.
To ensure the quality of client's room/ environment is of a high standard during each shift in charge.
To ensure that the 'key worker' system is organised and working during each shift in charge. To provide clients with high standards of nursing service in all aspect of their individual activities of daily living.
To develop a consistent problem solving approach to all clients activities of daily living as part of the multidisciplinary team.
To provide practical and emotional support for clients to promote, enable and expand their level of independence.
Working with the support workers in facilitating social and life skill opportunities and enabling community integration as part of client's holistic care, therapy and rehabilitation programme.
To develop good communication and rapport with staff, relatives, health care professionals, advocates, volunteers, and encourage family members/ advocates to participate in the rehabilitation programme.
To take charge of the unit and to understand the responsibilities relating to management in the absence of the Manager/ Clinical Lead.
To supervise, support and direct support workers to provide clients with high standards of nursing and care service in all aspects of their individual activities of daily living.
To provide clinical supervision/ teaching for staff as required. EG continence care, pressure care (as stated in N.I.C.E guidelines and practices), observations, note taking, airway management etc.
To ensure the safe custody, administration and recording of drugs in accordance with the Royal Pharmaceutical Society and the company.
To deal with complaints, incidents and accidents in accordance with company policies and procedures.
To encourage family members/ advocates to evaluate and make suggestions for the clients holistic programme of care, rehabilitation and lifestyle choices.
To use and promote a holistic approach to nursing and maintain clients identified 'care plan' records and documentation for future planning
