The purpose of Public Health is to improve health and social well-being through taking a population wide perspective for Public Health improvement, focusing on tackling inequalities. This is achieved through the responsibility of working with individuals/families/groups/partner agencies to identify health needs, raise awareness of health and social well-being and through enabling/empowering people to improve their own health.
Within the Health Visiting function of this role the post holder will be accountable for a caseload, a clinical expert, provide Public Health expertise, and be a key member of the integrated Primary Care team and other multi-disciplinary groups as required.
The team leader role within this post encompasses leadership and management of the practice attached/aligned community nursing team and other nursing staff groups, where applicable, linking into the management structure of the locality.
Registered Nurse with valid NMC Registration ,Registered Health Visitor / SPQ in Public Health Nursing ,Educated to/working towards/operating at Degree Level
