Home Support Matters is a highly respected home care provider working within Norfolk and Suffolk.

We are a friendly, supportive and hardworking team who proactively support a large range of customers within different sectors and with different needs. The Clinical Nurse is a valued member of this team.

We are looking for an experienced Community Nurse to support our clinical function and provide Case Management to our NHS Continuing Health Care customers who have complex needs. This is an exciting opportunity that will require the successful candidate to work autonomously, and flexibly within an ever-changing health and social care community.

The Clinical Case Manager role would best suit an individual who has community-based experience and is able to demonstrate they are able to manage their own workload. The role requires the individual to provide training and assessment of competencies of clinical skills and act as a clinical resource to the wider team within the company.

Essential Skills
  • Registered Nurse
  • Current NMC registration
  • Minimum of two years experience in nursing

  • Effective organisational skills and that ability to manage own workload
  • An effective team player
  • Flexible approach to working hours and practices
  • Good IT skills
  • Must be a car driver with own transport
