This post requires the post holder to have a PVG Scheme membership/ record. Fixed Term Contract for 5 Months

Job Purpose

Working within the East Lothian Integrated Rehab Service with patients who have been supported home by Discharge to Assess (D2A). Patients will have been identified as requiring short term support with activities of daily living and require ongoing functional rehabilitation at home. The goal will be to support early discharge from the acute hospital alongside enabling patients in returning to their previous functional baseline and negating the need for formal care packages.

The Band 3 Rehab Support Workers will work alongside Occupational Therapists, Physiotherapists and both Physiotherapy and Occupational Therapy Assistance Practitioners (Band 4). Goals and levels of support required will be pre-determined by OT and PTs from the Acute and also following initial assessment on day of discharge. These will then be fully discussed with Band 3 Rehab Support Workers.
