MidCo Care are expanding and are now looking for an experienced Care Co-ordinator to join our growing team. You will be responsible for growing the business and coordinating all aspects of the Domiciliary Care Service Business in their allocated region/s. The role involves implementing the support that service users require in order to promote their independence, choice and control. The goal is to provide excellent services that meet the social and health needs of the individual service users.

Primary responsibilies are:

Contribute to the growing of the homecare services by increasing the volume of care hours per week in allocated region/s.

Receive requests to provide home care services and liaise with the referrers to secure the work

To meet and maintain targets in relation to hours versus income, quality assurances, key performance indicators, CQC compliances and action plans

Establish care worker teams and coordinate their service provision with service users

Manage weekly rotas to ensure complete staffing at all times, coordinating, planning and liaising with care staff, social services and the next of kin of service users

Rostering and allocating hours to care workers, using the provided IT systems

Maintain the company rostering system to capture changes as soon as possible on the system

Maintain the holiday schedule, visit completion and time sheets for the payroll and accounting systems

Manage the on-call cover outside of normal working hours and maintain a detailed log of all on-call activity

Supervising care workers, conducting Supervisions and Spot Checks and other people management practices to meet Company and regulatory requirements

Liaise with social services, the NHS and private users and other healthcare professionals ensuring that we are effectively collaborating with other professionals in a timely manner.

To contribute to the development, establishment and maintenance of high Company quality standards, systems and procedures in line with CQC Compliances and Health & Social Care Act and contract compliance.

To attend relevant planned and unplanned; daily, weekly and monthly meetings and business reviews to be knowledgable of and familiar with current and new legislative changes within recruitment and the domiciliary care sector.

Work with clients to assess their needs and put in place the necessary care plans, risk assessments, and associated Company administration documentation to meet the needs of the business and service users.

Being aware of any changes in service users' needs impacting on Care Plans, and updating the plans to reflect the new needs and implementing the changed care.

To co-ordinate and manage issues and problems that may arise with clients and care workers and investigate / resolve where required seeing through to an appropriate conclusion.

In collaboration with compliance teams, recruiting, and on-boarding/inducting new care staff as and when required, ensuring consistent flow of good quality candidates

Producing timely and accurate weekly and monthly reports on the programme activities; and ensuring that all relevant data and programme forms are completed in line with Company and regulated standards e.g. all communication logs, MAR Sheets and charts are returned monthly and audited in-line with requirements.

Investigating medication errors, safeguarding incidents, other incidents and accidents, and any other issues, ensuring relevant internal and external reporting, including where applicable, to external institutions such as, CQC, local authorities, NHS and the police.

Carry out on-call duties on a rota basis with team.

Creating and maintaining a culture of professionalism, performance and excellence.

Providing cover for other services as may be required.

Carry out any other duties as may reasonably be requested by the line manager
