DCHS is a top performing NHS Trust and we have Community Nurse opportunities within the Integrated Community teams in Derby City (Coleman Health Centre, Village Medical Centre and aligned GP practice bases).

We are seeking Registered Nurses with or without community nursing experience to join our teams.

You will bring transferable skills, an enquiring approach and a willingness to work in an integrated way, truly embedded in our Integrated Community Teams (ICTs).

Community experience is desirable but we are confident we can support the transition into a community role.

We seek motivated nurses with passion and commitment who hold patient experience and care at the heart of their nursing practice and share our organisational values.

Clinical skills competence is desirable, but we will enable your learning through our robust Continual Professional Development package to fill gaps in skills, plus we actively support your career progression.

The role requires day to day delivery of patient care in our Integrated Community Teams alongside District Nurses, Senior Community Nurses, admission avoidance services, therapists and social workers .... together you make the difference.

We operate a shift pattern between 8am and 6pm, covering 7 days including Bank Holidays.
