Community Mental Health Nurse - Band 6/5

Dementia services Crawley and Horsham.

We have an exciting opportunity to offer the right person the chance to be a part of our committed and enthusiastic team working with people who have complex needs relating to their diagnosis of dementia. Our service is in the process of moving away from a more traditional model to adopt the follow me care principles advocated in NHS England's new Community Mental Healthcare Strategy. We are confident that this approach will help us to meet the growing demands for services whilst continuing to provide good outcomes for people.

This post will sit within our multidisciplinary community dementia team and will work alongside our lead practitioners to deliver interventions to those people who have more complex or intensive needs. We aim to help people to remain safe and well at home and able to maintain therelationships that are important to them.

The service operates 365 days a year between the hours of 8am-7pm Monday-Friday and 9-5 weekends and bank holidays. The successful candidate will be expected to work 10 hour shifts on a rota basis.

This post offers The Trust's Golden Hello for band 5 or 6 Nurses, or a Relocation package up to a maximum of £2000. (pro rata applies and subject to terms and conditions. Please see attached Protocol document).
