West Sussex Dementia Services Community Support Workers - Band 3 .

There is an exciting new opportunity to work with people with dementia, their family and carers - we are expanding our service to include more Community Support Workers to work within our Community Dementia Services across West Sussex.

We are looking for 2.5 Band 3 Community Support Workers to join our West Sussex Dementia Service in the Chichester and Bognor Area.

West Sussex Dementia Services are entering an exciting period of remodelling in preparation for the establishment of a Specialist Older Adult Service. As part of this work we are actively reviewing all of our existing teams in consultation with our staff, service users and carers. This includes Memory Assessment, Living Well with Dementia and Dementia Crisis teams.

These posts will offer intensive support interventions in the Community Dementia Service. The intensive support element operates an 8am - 7pm 10 hour shift system Monday to Fridays and 9-5 at weekends. The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible.

The base for this post will be Chichester.
