Lewisham has already successfully introduced clinical pharmacists into the GP practice setting with an integrated and collaborative model across primary and secondary care as part of the NHS England's GP practice pharmacist programme.

An exciting opportunity has arisen for a motivated and patient centred band 8a clinical pharmacist to work within the general practice setting as part of The Lewisham Care Partnership Primary Care Network (TLCP PCN). TLCP represent 5 local GP partnerships which merged in 2017 and now form a PCN with population of nearly 60,000 patients.

The postholder will also provide the clinical leadership to develop the PCN pharmacy service across participating sites to develop efficient and innovative models of both direct care and medication safety workstreams. This role will also involve working within GP practice settings to provide direct care including specialist advice to patients undertaking full patient-centred medication reviews to manage people with complex polypharmacy, particularly the elderly and those with multiple long term or chronic conditions.

Responsibilities will also involve partnership working across health and social care interfaces including close working with the wider Lewisham Integrated Medicines Optimisation Service (LIMOS) as part of further development of the integrated approach to medicines across the community, primary and secondary care settings and in line with the NHS Long Term Plan.

The pharmacists will follow an education and training pathway as part of the General Practice 5 year framework currently delivered by the CPPE, unless equivalent qualifications exist already.
