Complex Care Matron - Full Time, job share considered.

Leighton Road Surgery is a large, friendly, dynamic, training practice with a patient list of 21,000. We offer a complete spectrum of services and endeavour to put the patient first by adopting best practice. We have an opportunity for an experienced nurse to join our forward thinking and supportive team and shape nursing services for patients with complex health needs and those that are housebound.

We have had a challenging time, reflected in our recent poor CQC report however, we are optimistic about a future collaboration with ELFT and exciting times lie ahead.

The post holder will have continual responsibility for a caseload of patients with complex long term chronic conditions that will require advanced specialist nursing skills and assessments. You will be expected, in collaboration with the patient, and/or carer, to produce an individualised on-going patient management plan that may require periodic adjustments.

It is expected you will case find patients with chronic diseases, and proactively case manage these high risk patients with complex needs, multiple co-morbidities and long term conditions, to prevent unnecessary hospital admissions and to facilitate hospital discharge in collaboration with the integrated team. Also you will have the autonomy to act on behalf of the long term condition patients, making differential diagnosis, prescribing treatments, investigations and/or referrals.

In order to meet the role requirements' it is essential that you possess a high level of communication and interpersonal skills at inter-professional and inter agency levels to ensure that high standards of care pathways are maintained.

Car owner/driver is essential.
