Discharge Coordinator Band 5 Full Time (37.5 hours) Mon - Fri 9-5. An exciting new opportunity has arisen within our Community Hospitals (1 for each ward at Walton and Woking) to be part of a forward thinking and dynamic team to manage all discharges and coordinate all relevant services to ensure timely, appropriate and safe discharges as well as triaging referrals therefore supporting effective patient flow. In addition the successful postholder will supervise junior staff as well as assess, plan, implement and evaluate programmes of care to support the team as a whole. Recruitment - Golden Hello Successful applicants, who have not worked for CSH within the last 12 months, will receive an amount of £2000 paid in two instalments: - £1000 upon commencement and a further £1000 following successful completion of probation. These payments are pro-rata for part- time posts. Payments will be subject to tax and NI National Insurance in the usual way.We don't operate Golden Hellos for Band 7 or Band 8 posts, Bank posts or fixed term contracts.
