Communication & Relationship Skills
  • Act as point of contact for patients enquiring about their referral and how their referral is progressing through the Service
  • Liaise with clinicians, dental nurses and receptionists to help manage referrals within the Service and at the various clinics
  • Monitor and record patients through the agreed pathway, chasing members in the Service or patients to ensure that the Service is compliant with contracted quality measures relating to referral management
  • Monitor the sending of responses to referring practitioner about patients referred to the Service
  • Monitor referrals sent out from the Service
  • Develop and maintain relations with referring practitioners, providing details about acceptance criteria, updating them on progress of referrals as required and be a point of contact for advice and support to them
  • Ensure that referrers are kept informed of patients progress as required, including when patients have failed to attend an appointment and the outcome of a patient's course of treatment, etc.
  • Assist in the development of proforma letters to aid responses to referrers
  • Advise on how the referral system can be developed to ensure a better, more efficient service and patient outcomes can be achieved

Analytical & Judgemental
  • Check that relevant information as set out in referral criteria about the referral has been included within the referral, and where a referral is missing information, to either return the referral with information as to why the referral has been returned or seek out the additional information from the referrer
  • With the support of an appointed clinician, record whether the referral meets the relevant acceptance criteria
  • Produce regular reports on the number of referrals and the Service's management of them for each part of the Service, predicting the performance of the Service in managing any waiting lists

Planning & Organisational Skills
  • Receive and manage referrals meeting the admission criteria. Enter referral information on appropriate IT system to enable progress of referral to be monitored
  • Copy the referral letter and maintain a file of copies of referral letters for audit purposes
  • Responsible for patients to be contacted and appointments made for patients to be seen by the Service within the Service's accepted time frame, recording where patients wish to postpone or delay any appointments
  • Arrange appointments for patients to be seen, within the set time frame, by an appropriate clinician as specified by the triaging clinician
  • Where paper notes are maintained, ensure that patient record card and folder are made up and filed, including a copy of the referral and any radiographs
  • Provide information to the website manager to ensure that Service information is up to date on the website

Physical Skills
  • Use keyboard skills to enter referred patients' details onto the Service's patient management computer system including name of referring practitioner and patients' medical practitioner details
  • Scan details of referral into patient management system, eg letter and radiographs

Patient/Client Care
  • Responsible for patients to be contacted as needed to ensure they can attend planned appointments, sending out appointments and confirmation of appointments by post, recording where patients wish to postpone or delay appointments

Qualifications
  • Good standard of education
