£17,823 - £18,854 per annum
74 hours per week available (equivalent to 2 x 37 hour posts)*

Southdown has been awarded the contract to act as Lead Provider to deliver the Community Roots contract in Brighton and Hove from October 2019, working collaboratively with mental health providers to develop a network of integrated support. Services provided include prevention, wellbeing and recovery support for people with a range of mental health challenges. Support for people's carers and families is also available.

An exciting development for the services will be the introduction of a Central Access Point telephone number and website that people and professionals can use to help navigate and access services.

As a Navigator you will be part of a team who are the first point of contact for Community Roots, providing an effective interface for members of the public, partner organisations and other agencies.

Responsibilities will include:
  • Responding to all calls and electronic requests to the Central Access Point.
  • Using a person-centred and holistic approach to assess a person's needs, ensuring choices are offered.
  • Gathering sufficient information to ensure appropriate pathways for people are identified.
  • Giving advice and information, as well as signposting and facilitating access to partner organisations within Community Roots.
  • Ensuring all calls and referrals are recorded onto a database.
  • Maintaining an excellent knowledge of partner organisations.
