Purpose of the Role
The post holder will be responsible for the autonomous clinical assessment, treatment and/or referral or giving advice as appropriate to first contact clients across a wide spectrum of clinical conditions within the services provided by DHU Health Care CIC through direct contact consultations and by telephone. Direct consultations will be undertaken in the patient's normal place of residence or in clinic settings.
A commitment to a quality service, excellence in clinical practice and continuous professional development are integral to the role and the development of the service.
The post holder will participate in promoting and advancing DHU Health Care CIC.
Key Areas of Responsibility & Accountability
Clinical
  • To practice as a Nurse Practitioner / Emergency Care Practitioner without direct supervision but with support available by telephone.
  • To assess, plan, treat and refer patients appropriately and evaluate patient care.
  • To recognise the need for, initiate and where appropriate interpret relevant diagnostic investigations.
  • To recognise the need for and initiate where appropriate advice of relevant specialisms.
  • To exhibit excellent communication skills when dealing with clients. Non verbal and verbal skills required. To have the ability to make clear decisions with confidence and communicate these effectively.
  • To maintain appropriate clinical records, ensuring that documentation clearly demonstrates the NMC Guidelines for Records and Record keeping or Health and Care Professional Council for Emergency Care Practitioners.
  • To supply medication under patient group directives or function as an independent prescriber.
  • To work within the parameters of his/her skills and knowledge, to recognise the limitations of ones own practice and to work within the bounds of ones training and capability according to the NMC Code of The Code: Professional standards of practice and behaviour for nurses, midwives and nursing associates & Health and Care Professions Council
  • To maintain an awareness of developments in clinical practice.
  • To optimise evidence based practice within practice.
  • To work effectively as part of a multi professional team in collaboration with other members of the service.
Person Specification
The job holder will have;
Qualifications
  • First year level registered nurse minimum of 2 years post reg. Or Emergency Care Practitioner
  • NMC / HMPC Registered
  • Degree module in Injury and /or Illness
  • Experience of autonomous practice supported by appropriate degree/masters level study
  • Experience using advanced clinical examination & assessment skills within practice supported by the appropriate post registration qualification.
Experience
  • Experience of autonomous practice working and multi-professional team working
  • Can show evidence of using advanced physical examination and assessment skills in practice
  • Can demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children
  • Has experience in nurse led clinics or emergency care centres ,out of hours facilities or walk in centres
  • Experience of prescribing medicines under patient group directives
  • Experience in an Urgent Care setting either in A&E/WIC/UCC/First responder or similar.
Additional Information
Shifts could fall between the following working hours, however this would be discussed further at interview;
Westcoats - Mon - Sun - 8-8pm,
Merlyn Vaz - Mon - Sun - 8-8pm,
Brandon & Saffron - Mon - Fri - 18.30- 22.00, Sat - Sun - 12.00-20.00
