We are looking for an enthusiastic, highly motivated and well-organised Band 5 nurse or therapist to work in the Referral Management Hub in Brighton; this supports all the referrals to Brighton and Hove responsive Services and the rehabilitation beds in Brighton.

The role involves the triage and co-ordination of referrals that support early hospital discharges and avoidable hospital admission referrals can come from hospital, GPs, paramedics and other agencies.

The post holder will be required to work as part of a team, but also independently, prioritizing referrals using clinical decision making skills, seeking support from senior staff to manage complex referrals as required.

The post-holder will ideally have an understanding of the various Sussex Community NHS Foundation Trust services and be able to effectively communicate with a wide range of people including clinician's, therapists, social care, managers, staff and patients using the services.

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
