We are seeking two professionally qualified assessors to develop and manage the scheme in a multi-disciplinary environment

Salary; £30,400-36,500

Overview

Following a period of in-patient care, and prior to an individual's discharge, the Trusted Assessor is required to undertake an assessment or review to determine eligibility and requirements on behalf of care providers. The assessment is also required for individuals who are discharged to enable a safe discharge and smooth transition.

Historically in Somerset, care providers have conducted their own assessments of individuals in the hospital setting, prior to a home discharge being made and implemented. An assessment requires adequate time and effective dialogue between the individual, the care provider and the hospital team.

The Trusted Assessor role is to provide the necessary additional resources to support the care provider in conducting a timely and thorough person-centred assessment that meets the needs of both the individual and the care provider.

Accountability

The post holder is accountable to the RCPA as employer via their line manager.

The post holder is also responsible for providing regular reports to the TA Steering Group

The RCPA will support the post holder with any re-validation requirements for professionally qualified staff.

Work base

The post holder's workbase will be at either Musgrove Park or Yeovil District Hospital.

There is a requirement to attend both hospitals to ensure adequate cover.

The post holder will be expected to travel between their workbase and hospitals, care provider addresses and other venues for meetings.

Key Purpose

* Lead on the delivery of the TA scheme as a resource for care providers in conducting pre-discharge assessment.

* To implement the pilot scheme, with a view to replicating it across the county

* To develop and implement a county-wider TA model that contributes to timely transitions for individuals back home, from acute hospital settings and hence the effective delivery of the patient pathway and positive outcomes for individuals

Main tasks

* To develop and implement a 12 month pilot of the scheme and in conjunction with the Steering Group, evaluate its effectiveness with a view to expanding the scheme across all provider types within Somerset to undertake assessments and reassessments on behalf of care providers in order to facilitate safe and timely discharges

Establish and maintain effective and trusted collaboration with care providers to ensure that effective and thorough assessment are delivered on behalf of care providers

Based on assessment, make recommendations to care providers discharge decisions.

Develop effective relationships with patients and hospital discharge teams and clinicians to promote choice and good outcome for individuals being discharged home following in-patient treatment.

Work with care providers and hospital discharge teams to develop jointly agreed protocols and procedures for discharge assessment ad decision-making

* Work with care providers to explore the potential for developing generic assessment tools

* Work with care providers to identify challenges and obstacles to timely discharges and seek way to address these

* Keep all assessments undertaken for the purposes of quality control and evidence in electronic formats and disseminate to appropriate staff in Health or Adult Care

* Promote and raise the profile of the role with care providers through appropriate channels

* Collect and complete accurate and timely recording of performance and delivery to support evaluation of the scheme and reporting, including that from individuals (patients and families) using the service.

* Manage own administration

* Engage and be accountable via regular supervision and line management processes and contribute to appraisal and development opportunities with the RCPA

* Attend TA Steering Group and other meetings as appropriate

* Adhere to RCPA organisational and HR policy and procedure

* Be responsible if and when necessary for the recruitment and line management of junior staff employed for the purposes of the scheme
