Sussex Community NHS Foundation Trust is looking for an enthusiastic and compassionate registered nurse to join our multidisciplinary team on Harlands Medical Ward at Uckfield Community Hospital.

We are a 14 bedded Intermediate Care Unit which provides high quality care for patients with a variety of medical needs including semi-acute admissions, rehabilitation, palliative and terminal care. We care for patients both 'stepping down' from secondary and tertiary care and 'stepping up' from home to prevent admission to secondary care. We aim to provide a service that supports patients to maximise their ability to live independently and return home when it is safe for them to do so. Our staff are skilled in providing palliative and end of life care, having close links with St. Wilfrid's Hospice. Harlands Medical Ward is some patients' chosen place for end of life care.

As a staff nurse on the ward you will play an important part in the patients' recovery. Working as part of the multidisciplinary team, you will have the opportunity to use and develop key skills, providing holistic assessment and patient care. You will be responsible for contributing to care, observing, monitoring and recording patients' progress whilst communicating and managing effectively any changes in their condition and planning their safe discharge. Liaising with external partners and patients' families is a very important part of the role.

You will need sound, up to date clinical skills, be reliable and have a flexible approach with a proactive leadership style. Uckfield Hospital has free on-site parking facilities which can be used by staff.

The post-holder will be required to work flexibly on both day and night shifts. Full/Part-time hours/flexible working patterns considered.
