An exciting opportunity has arisen for an experienced Staff Nurse with excellent assessment and communication skills to join the Sussex Community NHS Foundation Trust, System Capacity & Flow Team in Brighton and Hove as an Assessor. The main focus of the role will be assessing people on a needs-led basis who have been referred for a rehabilitation bed. These assessments take place both within the hospital, on the wards to support discharge and in the community, to prevent unnecessary admission to hospital.

The role will also involve liaising with the Registered Managers of the homes supporting Brighton and Hove City Beds, ensuring that the Registered Managers are fully informed of the person's needs prior to acceptance and agreeing their admission.

You will be well supported by the senior clinicians within the team, and learning opportunities are in place for your continued professional development.
