I am working on behalf of a very reputable Domiciliary Service provider that requires an experienced Care Coordinator for their Cardiff based branch.

The Company:
A forward-thinking Home Care provider, dedicated to providing person-centred care services to older people, adults with physical, mental health needs and adults with learning disabilities. They work together with the local health professionals, PCTs, different parts of communities and charities with the aim to provide a holistic service to all our clients in order to allow them to remain independent in their own home for as long as possible.

The role:
This particular branch provides 3500 hours of care per week. They are the largest branch nationwide within the company. They are a friendly, passionate and continually evolving home care branch, who's main purpose is to deliver high quality care to their clients.

You will duties for this role include:

* Ensure the highest possible levels of care provision are delivered and maintained at all times following all Right at Home policies and procedures
* Ensuring care is delivered and we, as a team and as more senior individuals, assist with covering short notice needs/calls. This isn't ?planned? but ?unplanned? activity due to emergency situations but will not impact on the ?time off? element of a Supervisor's working week (unless they volunteer to do so).
* Have an eye for Quality at all times and report any quality issues observed in the community immediately.
* Contact the office when potential risks are noticed and also ?reportable? health and wellbeing concerns e.g. red pressure areas, changes in risk, changes in client conditions and/or mobility etc.
* Contact relevant parties on behalf of the company e.g. District Nurses, GP's, Family as requested by your coordinators or ?on call? personnel.
* Support and contribute actively to the training, shadowing and supervision, including spot checks, of existing and new staff in all aspects of their work.
* Be there to mentor and coach new starters giving them the best possible start to their career in care and introduce yourself to all new starters during Induction Training weeks.
* Promote the company policies and procedures to new starters.
* Support them as required and be their point of contact out in the community.
* Support and mentor a team of Caregivers on a day to day basis to ensure they perform to Right at Home standards of quality.
* This includes bringing carer issues/misgivings/concerns to the office in a timely manner so that we can act on things quickly and also to offer additional support.
* Liaise closely with the Registered Manager/Assistant Manager and Care Coordinators at all times, reporting any concerns, changes or difficulties regarding staff or clients.
* Following your supervision sessions contact the office and report any points noted : positive or constructive so that we can act of things quickly. Arrange to come into the office on a regular basis to communicate information, deliver paperwork/documentation and generally catch up with your coordinator and/or other relevant parties based in the office.
* Ensure a thorough knowledge, understanding and adherence to all company policies and procedures in line with CIW regulatory requirements.
* Ensure that your carers and your clients are compliant and act on any compliance e-mails from the Compliance officer to ensure we are up to date.
* Participate and arrange team meetings and staff events to ensure the effective communication of business goals and objectives on a regular basis.
* To attend formal group Meetings at the office once every 2 months.
* To arrange meetings with office personnel as and when required in the interim.
* Closely monitor and maintain clear, timely records such as communication sheets and MAR Charts, alerting the office to any issues that may cause concern.
* Liaise with Jemma in Compliance on any concerns and ensure that documentation is collected and delivered to office for further investigation if requested.
* Closely monitor and maintain house files ensuring they are fit for purpose and that all information is current, relevant and up to date and take action accordingly and report to office.
* Keep in touch with your coordinator to ensure that any files that need updating, replacing etc. can be put together in a timely manner.
* Deal with all enquiries in a calm, friendly and professional manner to ensure you represent Right at Home effectively.
* We require a positive Field Care Supervisors and we want to work together as a team
* We are all ambassadors of the company and the brand and we always need to be professional.
* Assist and conduct client assessments and reviews as and when required including care planning, risk assessments, moving and handling assessments etc. (Full and continued training will be provided ongoing).
* Attend training sessions planned
* Seek advice and guidan Field Care Supervisor, Domiciliary, Elderly, Dementia
