We are the largest provider of NHS services across the County of Cumbria. Around 3500 staff operate 60 community and mental health services from over 20 main sites and many other premises shared with other health or community services such as GP surgeries. In any one year we see around one fifth of the population providing health care services to people in Cumbria, from newborn babies to care of the frail and elderly. Our services are organised into care groups, designed to put clinicians in control of decision making with patients and carers so that services are run by experts in the clinical area.

Home Care Practitoner - Permanent

We have an exciting opportunity for you to join the Eden Integrated care Community Team as a Home Care Practitioner, based in Eden or Alston.

We are looking for motivated and enthusiastic Home Care Practitioners to work within our Community Health Team in Eden.

You will provide valuable short term support to people who have had a period of ill-health, helping them to return to independence. This will include help with activities of daily living, personal care and monitoring the patient's condition, using an enabling ethos, promoting self care; and delivering input as needed.

You will play a key role in preventing hospital admissions and facilitating safe, supported discharges from hospital, working closely with Health and Adult Social Care colleagues.

Effective communication both written and verbal is essential.

Your work will be directed by a Team Lead; but you will work unsupervised as a lone worker.

Health and social care teams are working together to improve the overall wellbeing of local people by joining up health and care services, providing more care out of hospital and supporting people to manage their own health. Health and care professionals, GPs, the voluntary sector and the community will work as one team to achieve this. NHS organisations together with Cumbria County Council, District Councils and the community, voluntary and independent sectors are committed to making changes that put Cumbrian people at the centre of our health and care services and this will largely be done through Integrated Care Communities.

Applicants will be expected to have a commitment to working in this area and be able to demonstrate the ability to work within a changing environment with a flexible approach and be keen to develop as the service demands. The shift pattern may be split shifts working 08.00-13.00 and 16.00-21.00 but may alter to work between 0800 and 2230, seven days per week, and work can be across anywhere within the locality (and out ofthe immediate area if requested), therefore access to transport is essential.

The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

Standard Check: £26 + Administration cost of £4.20 = deducted at £10.20 from 1st month salary and £10 deducted from salary over following 2 months.

Enhanced Check: £44 + administration cost of £4.20 = £15.20 deducted from 1st month salary and £11 deducted from salary over following 3 months.
