An exciting opportunity has arisen within the busy but friendly Out of Hours Enhanced Care Team.

You will be joining our dynamic and supportive Enhanced Care Teams to provide a 24 hour admission avoidance service to the population of Worcestershire, which encompasses both urban and rural areas. This post will enable the service to offer an increased night sitting service, for vulnerable patients in their own home.

The successful candidate will deliver a high standard of care for patients in their own homes. You will be expected to support a patient on a 1:1 basis, during the night ensuring that care provided maintains the individual's privacy and dignity, values diversity and respects cultural and individual rights and choices.

We are looking for candidates with good communication skills, a flexible approach and enthusiasm to work within the team. You should be a committed, highly-motivated and forward-thinking individual who is a good team player, but also happy to work alone in a community setting. Ideally you will have recent community experience within the NHS.

It is essential applicants have experience of working with older adults in a health/social care setting. A Level 3 qualification (e.g. A Level, NVQ3) in Care or a Level 2 qualification (e.g. GCSE Grade A-C, NVQ2) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post.

Successful candidates will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.

The successful candidate will assist the team to develop and achieve team and organizational objectives. They will also participate in staff meetings and training & development activities as necessary to ensure their skills and knowledge are up to date.

Due to the nature of the post the applicant must be able to travel independently and have access to a suitable vehicle for business purposes. Whilst an appropriate base will be agreed at recruitment across the county, staff can be asked to work anywhere within the county or the Business Unit or Service if necessary.

The Trust offers a preceptorship year for newly qualified nurses and AHP's and attendance at preceptorship programmes is a condition of employment.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Tier 2 Certificate of Sponsorship
