We provide a comprehensive range of secondary care services from 3 main centres, Grimsby, Goole and Scunthorpe as well as community services in North Lincolnshire.

The Single Point of Access Service which will incorporate the Virtual Ward is continuing to develop and change in order to meet the needs of the local population. The service is undergoing expansion and we now require an additional 6 full time equivalent registered staff nurses. The hours are varied and include a mixture of shift patterns and will be rotational to cover a 24 hour, 7 day a week service need. The posts could be a mix of full-time and part-time staff and if you have a preference for the number of hours you would wish to work please state this in your application.

These posts will suit staff nurses who enjoy working within a developing service and are motivated and enthusiastic.

The purpose of the Single Point of Access Service (SPA) is to signpost patients who require health and social care services to the most appropriate service that meets their needs.

The SPA service aims to:

  • Offer an alternative to secondary care where safe and appropriate


  • Assist in the reduction of the length of a hospital stay by ensuring patients are signposted to the most appropriate place


  • Assist in the management of patients accessing the GP Out of Hours Service which includes receiving and managing referrals from NHS 111


  • Provide a telephone triage service and/or a secondary assessment for patients who have ongoing health needs and signpost to the most appropriate service or provide self-care advice


The virtual ward describes an enhanced package of healthcare provided with the aim to maintain care within the home wherever possible. It is an alternative pathway for those patients:

  • who do not require acute medical in-patient support and are clinically fit for discharge, but do require on-going nursing interventions and care and/or intensive therapy


  • where admission to an acute hospital can be avoided through the provision of intensive therapy and/or nursing interventions and care


NB Please note the job description attached does not include rotational working and there is an expectation that the staff employed will be employed on a rotational contract.
