Health Care Support Worker - Band 2

22.5 hours per week - Permanent. - One post

A post has arisen to work within the South Holland ICT

You will provide a high quality service to people within community settings working closely with all members of the multi-disciplinary team. You will undertake delegated nursing tasks to provide personalised care interventions to people and support an enabling approach to maximise independence.

You will be required to work flexibly between the hours of 0900 and 1900, seven days a week with a flexible approach to shift working.

A minimum of 2 years experience as a community or hospital health care support worker, preferably with NVQ level 2 or equivalent is desired.

Possession of the 'Care Certificate' or a willingness to work towards obtaining it is essential.You must possess excellent communication and interpersonal skills and the ability to work across health and social care boundaries.

Additionally you will need to demonstrate IT literacy and this post requires that you have the ability to travel between locations to carry out your work and to attend training and meetings; daily access to transport is therefore essential
