If you are an efficient worker with strong clinical knowledge, excellent communication skills and able to work under pressure whilst remaining focused on safe good quality care, seeking a new personal challenge we look forward to your application.

We are looking to recruit a 37.5 hour post within the Medical Care Group working over 5 days a week (Monday to Friday). You will need substantive years post registration experience and a sound clinical background and a valid NMC registration are essential in this role. We are seeking an enthusiastic, knowledgeable and self-motivated Band 5 with a 'can do' mindset and a strong interest in patient flow.

If successful you will be part of a fantastically motivated and forward thinking team who constantly strive to give the highest standards of care which involves our patients being discharged in a timely manner.

You will work closely with our ward co-ordinators to identify discharges and to ensure everything is in place for the patient to move safely to the transfer lounge and then home, through liaison with our partnership social workers, care homes and families.

You will represent the units at our Trust discharge meetings where we highlight issues with discharges and we work with the CCG to escalate and troubleshoot delays within social care which prevent our patients leaving hospital when medically fit for discharge.

The post holder should have up to date knowledge of current issues in patient flow and changes within the NHS. The role is challenging and instrumental to the day to day operations within the Trust.

This post will be subject to an Enhanced DBS (with barred list for vulnerable adult's and children's disclosure).
