About you:

You are an enthusiastic and experienced District Nurse Specialist Practitioner, with excellent communication and people skills. With a passion for patient-centred care, you will facilitate your community nursing teams in supporting patients in their preferences, helping them to self-manage their conditions and empower carers to participate in treatment plans. Active on the mentor register, you will be an inspirational leader, supporting and motivating your teams and creating a culture of lifelong learning and development. Collaborating with a diverse range of teams and external agencies to ensure that patients and carers receive a seamless service, you will have a creative and innovative approach to service development. Persuasive and adept at managing change, you will be an ambassador for our service, promoting a positive image of community nursing.
