Humber Teaching NHS FT now delivers Community Services in the Scarborough & Ryedale area. This is an exciting time to join our teams as we introduce new models of care based around locality hubs. There are 3 hubs based around clusters of GP Practices, Ryedale, Scarborough North and Scarborough South.

An exciting and rewarding opportunity has arisen for organised, motivated and proactive Band 4 community associate practitioner to join our community team within the Scarborough North Hub. Previous experience of community working and knowledge of system one is desirable but not essential.

The services are delivered 7 days per week, 24 hours per day. It is essential that you are able to transport yourself and equipment across the locality to visit patients in their own homes.
