An exciting opportunity has arisen for a Nurse Practitioner/Emergency care practitioner to join DHU within our Home Visiting Service. The Home Visiting Service is integrated with the LLR clinical navigation hub, night community nursing service. The aim of this service is to reduce inappropriate admissions to hospital by providing a rapid response injury and illness home visiting service for non-ambulatory patients who, if they are not visited rapidly are at risk of admission or attendance to secondary care.

What you will do;
You will be responsible for the autonomous clinical assessment, treatment and/or referral or giving advice as appropriate to first contact clients across a wide spectrum of clinical conditions within the services provided by DHU Health Care CIC through direct contact consultations and by telephone. Direct consultations will be undertaken in the patient's normal place of residence, with occasional working in clinic settings.
The successful candidate will likely have;
First level registered nurse or Paramedic minimum of 2 years post registration.
Evidence of undergraduate or postgraduate level module in advanced clinical assessment / minor illness / minor injury.
Experience of autonomous practice
Evidence of experience using advanced clinical examination & assessment skills within practice supported by the appropriate post registration qualification.
Evidence of using advanced physical examination and assessment skills in practice
Demonstratable broad and sound knowledge of complex and common conditions in primary care in both adults and children
Experience in nurse led clinics or emergency care centres ,out of hours facilities or walk in centres
