This is an opportunity to join an Edinburgh based charity that has supported older people in the community for more than 75 years. They empower and enrich the lives of older people to live life to the full. They provide expert dementia care and other care services across the city

Outreach is their registered care at home service. They don't believe in rushed, 15 minute home visits. They offer nothing less than one hour with any of their Outreach services.

As part of a dedicated team, you will provide a personalised and responsive care service for clients and carers.

Key Responsibilities
- Demonstrate a strong commitment to providing high quality person centred care: promoting social, physical, emotional and intellectual wellbeing.
- Liaise with the Service Co ordinator on all issues to do with client care and carer support.
- Work with individual clients and carers assigned to you and implement and review care plans.
- Ensure care plans, visit reports and risk assessments are recorded accurately.
- Meet best practice standards of service delivery.

Skills and Experience:
- Must have the minimum SVQ 2 in Health and Social Care or working towards such a qualification.
- PVG Scheme membership.
- SSSC Registration, when required.
- Full UK drivers licence would be an advantage.
