Cromwell Primary Care Centre, Grimsby and Laceby Village Surgery
SALARY: Dependant on experience and qualifications.
Due to retirement, the practice is now advertising for a Nurse Practitioner to join our friendly practice team. The role is for up to 20 hours per week, working over a minimum of 3 days. The role would predominantly include attending to telephone triage, seeing patients with minor illness, chronic disease management, etc. Application from candidates who wish to develop their skills in all of the above, will be considered as well.
The practice operates from 2 sites, the purpose built Cromwell Primary Care Centre in Grimsby and also having a branch surgery in Laceby Village, just a couple of miles outside of Grimsby.
We are looking for a dedicated and hardworking nurse to become an integral part of the current team which consists of 4 Doctors (full and part time included), a highly qualified practice nurse, health care assistant and a complement of clerical support. We are also supported by wider area teams such as district nurses and community matrons.
The practice is actively involved in the local health community and with the CCG. We achieved The Quality Practice Award from the RCGP in 2014. We were also rated as “Good” in our CQC inspection, early 2015, with an outstanding rating in care for the elderly. High QoF Achiever. We have a substance misuse clinic which is an opportunity to expand or train your skills in this area. We are also part of a local federation of 13 GP practices.
