They currently have a fantastic opportunity for a Health & Wellbeing Facilitator to support customers by developing activities designed to improve and maintain their physical and mental wellbeing, with a view to creating customer-led sustainable groups to do the same (developing social capital), to champion local and national health and wellbeing initiatives.

You will deliver support services which promote community inclusion and sustainable housing solutions, assist in the marketing of services to secure new customers and implement agreed actions on customers support plans.
To be successful you will have:

Previous experience in delivering services that promote independence to older and younger people
Previous experience of working in partnership with external agencies and colleagues
Previous experience of conducting needs and risk assessment and support and risk management planning
Some experience of delivering activities to promote health and well-being
Some experience of raising funding for activities

The successful candidate will have Level 2 English and Maths and you will have an understanding of local social care and health structures and/or strategies.

You will be expected to work flexibly to meet the needs of the service and work some evenings and weekends.

An Enhanced Disclosure and Barring Service (DBS) check; a full and current driving licence and the use of your own vehicle will be required for this role.
