Health Lead Worker - Haringey
(Fixed-term contract up to 31 st March 2019)
£26,025 - £29,519 per annum
37.5 hours per week
Haringey Local Authority have awarded our client and Thames Reach funds to work in partnership on a community based project that will have 740 beneficiaries over 2 years as part of the wider DCLG Rough Sleepers Project. They are the lead provider of Haringey's supported housing pathway accommodation and co-delivers the drug and alcohol recovery service.
They're looking to recruit a Health Lead Worker on full-time contract up until 31 st March 2019. As the post-holder you will deliver person centred client facing interventions across Haringey Health Services; will establish and maintain close working partnerships across community services and will contribute to the design of and data collection for a comprehensive service evaluation. Co-located between Haringey's Drug and Alcohol services and North Middlesex hospital you will work in partnership with health partners to prevent rough sleeping or help people rapidly exit it.
Across Haringey's Drug and Alcohol services you will lead on providing a holistic housing assessment and follow up interventions for clients who present as NFA ensuring these actions compliment planned Drug and Alcohol treatment.
You will advocate for and be part of a planned approach to hospital discharge to ensure that no vulnerable homeless person in Haringey is ever discharged without a clear plan in place. You will also provide specific support to and advocacy for clients on their accommodation options, possible reconnection as well as appropriate health, welfare advice and the initiation or reinstatement of care and support packages.
