The Role

We are recruiting the Team Manager for our Reablement and Assessment Care Team (REACT). This services’ main aim is to work alongside people in regaining their physical well being and relearning the skills necessary for daily living, with intensive support over a 6-8 week period. This includes aspects such as personal care, medication and support with cooking. Our staff team help to maximise long term independence meaning that people get to remain in or return to their own homes and live their lives as independently as possible. It's key for this role that your focus is on the enablement and empowerment of the people you work alongside.

To be successful in this role, you must feel comfortable managing and assessing risk, following safeguarding processes, meeting health and safety requirements and ensuring record keeping is accurate, reflecting current standards as required by CQC as well as supporting others in your team to do the same. As a Team Manager you will be responsible for ensuring that the team deliver quality care and support to all clients throughout their time with us. You will lead on the recruitment, induction, ongoing support and training of your staff team to get the best from them, including mentoring, coaching, providing supervision and addressing any performance issues, and you will be expected to consistently role model anti-discriminatory practice and positive values which underpin all aspects of service delivery. A requirement of this service is the use of an electronic call monitoring system, CM2000, to plan client visits and meet contract requirements - if this isn’t something you have used before, full training will be provided but some experience would be beneficial.

About You

The ideal Team Manager candidate will be approachable, friendly, professional and supportive, with excellent communication skills and an appreciation of different communication styles in order to successfully work in partnership alongside the Community Independence Team, other professionals & family members. You will also need to work using your own initiative, adopting creative and innovative approaches where required and in line with current regulations and legislation applicable to the service. Experience of working with people to regain their independence is an absolute must. You need to be a car driver as you will not only be out and about with people in their own homes and communities, but you also need to visit local hospitals and other medical professionals such as physiotherapists as part of your daily duties.
