Care Co-ordinator

Tailored Care Ltd offers an exciting opportunity to join our well established and fast growing Shropshire branch. The role requires a dynamic and flexible approach and a drive to succeed. Experience is required in a similar role and you will need a full driving license and your own transport.

Requirements:

  • MUST have experience on the Cold Harbour Computer System

  • Driving license is essential

  • Main Tasks and Responsibilities

  • To manage care provision

  • To ensure care plans and risk assessments are updated on a regular basis in line with internal policies

  • Ensure updates are recorded on the system

  • To update and monitor all relevant aspects of the Electronic Call Monitoring system

  • To assess, negotiate and decide on the feasibility of new packages from any contracting source

  • To ensure that quotations are raised accurately

  • To ensure that clients details are entered accurately

  • To discuss and negotiate with clients and/or relevant advocates and agencies about any situations regarding clients and the company

  • To negotiate with care staff regarding their rotas

  • To establish the hours that care staff are available to work

  • Prepare rotas to ensure the needs of all clients are met

  • To ensure that care staff receive their rotas in a reasonable and agreed time

  • To ensure that care staff have adequate travel time and breaks

  • To carry out appropriate and authorised investigations into complaints or incidents arising from the provision of care and to update the system appropriately

  • To always act in a manner which respects the confidentiality of clients, staff and any other parties

  • To carry out on-call duties - early mornings, overnight and weekends on a rota basis as determined by the Branch Manager

  • When on-call to provide telephone support and assistance to care staff and clients and to be available to be called out as necessary

  • To carry out appropriate and authorised investigations into alleged misconduct or poor performance of care staff

  • To enter all relevant information onto the system accurately and efficiently

  • To participation in the general management and administration of the company as required

  • To attend meetings as and when required to do so by the Branch Manager

  • To work flexibly as required by the needs of the service

  • To carry out scheduled/unscheduled quarterly supervisions and annual appraisals for CHA’s and implement any actions arising from these meetings

  • To ensure all meetings are recorded on TCP

  • To carry out spot checks on CHA’s and implement any actions arising from these checks

  • To attend and participate in relevant training sessions

  • To demonstrate a commitment to personal development
