Community Support Navigator - Integrated Healthcare in Custody and Wider Liaison and Diversion Service (IHCWLDS) – Support Navigation Team
Cheshire & Greater Manchester Community Rehabilitation Company (CGM CRC) are seeking to recruit three Community Support Navigators to work alongside our partners Mite Care & Custody Health and Northwest Boroughs Healthcare NHS Foundation Trust, in delivering the Integrated Healthcare in Custody and Wider Liaison & Diversion Service (IHCWLDS).
This service brings together police custody healthcare and wider liaison and diversion services. We ensure service users of all ages receive an assessment at key points within the Criminal Justice System; from police custody through to the court system and into the wider community. This service supports individuals with a wide range of health problems and vulnerabilities including mental health issues, learning disabilities and substance misuse with a view to reducing re-offending and improving individual lives.
We are looking for people with passion and empathy, who can support individuals in their journey away from offending. Working in a team of ten across the Greater Manchester area, you will provide brief, but focussed support for individuals who have been in contact with the Police. You will show an understanding of the issues that they face, how these can be overcome and a willingness to tackle any barriers that may exist. You will also know that any contact you have with a service user is an opportunity for change.
Full training for this role will be provided.
We would particularly encourage and welcome applications from people with “lived experience” of the challenges and services that some of the individuals that we work with will experience. This could include previous personal experiences in relation to mental ill health, substance misuse, offending or homelessness (though not limited to these).
CGM CRC supervise c11,000 offenders annually, across Cheshire and Greater Manchester. We are part of Purple Futures, an Interserve-led partnership of private sector, charities and social enterprise, that provides probation and rehabilitation services on behalf of the Ministry of Justice.
Use an outreach approach to assist service users to attend first key appointments.
Enable individuals to develop confidence in engaging with relevant services.
Enable individuals to develop life skills.
Promote independence in individuals who may struggle with everyday life activities.
Enable individuals to find out about and access appropriate services and facilities.
Promote service users’ equality, diversity and rights.
Work collaboratively with partners within the Integrated Health, Diversion and Liaison Partnership (IHDLP) to deliver effective interventions for all service users
Work collaboratively with your team and partners within the IHCWLDS to meet both contractual and performance targets
Provide high quality and effective interventions for all service users.
Comply with, and contribute to, children and adult safeguarding duties, promoting the welfare of children and adults and alerting relevant colleagues and stakeholders should any risk be identified
Perform other duties commensurate with the post as from time to time may be determined by the line manager, Director or above.
Ensure that all service user related activities will be undertaken in an enabling manner in line with rehabilitation goals to enhance consistency in delivery
The service has been co-commissioned by the Greater Manchester Combined Authority and Greater Manchester Health and Social Care Partnership.
