Pullar House, 35 Kinnoull Street, Perth

A permanent vacancy has arisen for a Social Worker in the Early Intervention and Prevention Team, 36hrs per week.

The successful applicant will assist in maintaining vulnerable people in the community by providing a range of social work and care management tasks. These include carrying out Outcome Focussed Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages. Responsibilities will also include statutory work under the Adults with Incapacity Act and the Adult Support and Protection Act.

The successful applicant will also have responsibility for providing a specialist resource within Community Care through the development of knowledge and experience in care areas of practice and to carry responsibility for cases with a high level of need, the complexity of which social work experience, knowledge and skills.
