The Client,
My client, a nationwide home care provider require an experienced care manager to work as part of a team providing high quality care & support to Clients so they can remain independent in their own homes. To participate in managing and developing the General Home Care service and maintain the aims and objectives of the organisation.
Working directly with the service manager this is an ideal opportunity for an experienced care manager to gain more experience and progress.

Purpose of Role:
To develop and manage General Home Care Services whilst ensuring the mission, vision and values of the organisation are upheld.

Duties of Role:
respond to new enquiries quickly and give referral all relevant information and send out information as required
- To assess client care needs and complete all necessary risk assessments, care plans and documentation
- To set up care packages and work with coordinators to ensure that the correct carers are matched to client needs
- To review care packages every 3 months and document and carry out ad hoc reviews as necessary
- To respond to changing care needs and support the care teams to fulfil their role
- To build relationships with family members and the multi-disciplinary team to ensure holistic support is provided
- Ensure clear communication and liaison exists with all relevant allied health professionals; with all relevant parties kept up to date with changes.
- To support the recruitment and training processes.
- To work with the Service manager and deputy manager in GHC to build and implement the service strategy
- To adhere and support care teams to adhere to all policies and procedures
- Ensure Health and Safety is maintained throughout all activities
- Ensure CQC standards are maintained through all activities

Person Specification
To be considered for this position ALL candidates must:
- Have a proven professional background in assessment and management in the care sector.
- Hold a desirable Professional qualification i.e. Nurse, Social Worker, Physiotherapist, Occupational Therapist
- Previous Training Experience
- Strong Interpersonal skills and the ability to motivate others
- Ability to lead a team
- Ability to work unsupervised
- Excellent organisational skills
- Ability and skills to deal with complaints as per policy and procedures
