The aim of Community Connectors is to bridge the gap between statutory agencies and people in the community as part of the Early Help Programme. This senior role is responsible for supervising the day to day operation of the Social Prescription Service, which will identify and support individuals and families in order to connect them with their community and/or appropriate services.
You will be required to offer support and advice, gain the trust and confidence of families and provide practical assistance and will need an excellent knowledge of the local community and the services available within the area. Using this knowledge and working closely with a wide range of partners, including GPs and other healthcare professionals, will allow you to ensure local people can access the right services at the right time.
In this role you will also contribute to the development of early help and prevention, as well as supervising a team of Community Connectors.
