Support Practitioner/ Support Worker

Salary £18,000 (pro rata'd)

Permanent, 22.5 hours per week

Our client is looking for a Support Practitioner for a supported accommodation project in Stoke-on-Trent which supports people with mental health, substance misuse and complex needs.

Working as a part of an innovate team you will deliver individually tailored support, engaging customers in specific activities and developing alternative strategies to manage the triggers which affect their ability to live a life that is healthy and safe and allows them to achieve their personal goals.

Main duties will include:

- Arranging and running one-to-one and group activities and involving external partner agencies.
- Carrying out regular 'safe and well' checks and health and safety tasks.
- Keeping customer case files and staff communication logs up to date on every shift.
- Providing emotional and practical support for customers in crisis.
- Prompting customers with daily tasks such as self-care and communal responsibility.
- Identifying and reporting maintenance issues and dealing with contractors.

To carry out this role you will need experience of supporting adults with complex needs, along with:

- The ability to respond appropriately to challenging behaviour and de-escalate situations
- Experience of carrying out risk assessments and developing risk management plans
- A clear understanding of professional boundaries, vulnerable adults and safeguarding
- The ability to record and present information accurately and at the right time
- The ability to use Microsoft Office and other IT packages
- Resilience, a positive attitude and a willingness to work flexibly
- A full driving licence, access to a vehicle and willingness to travel locally, including transporting customers would be beneficial but not essential
