Pay ranges from £16.00 - £26.00 p/h for ltd workers.

Your key roles and responsibilities will include clinical skills such as:

* Collecting daily list of patients from district nursing base.

* Visiting patients in their own home.

* Maintaining exceptional levels of care.

* Liaising with Team Leaders to ensure duties are complete.

* Assessing and talking to patients about their problems and discussing the best way to plan and deliver their care

* Establishing robust communication networks with the client, their carers and other members of a multi disciplinary team

* Give sound and professional clinical advice and knowledge where appropriate

* Promote service user empowerment

* Effective delivery of care that promotes choice and independence

* Building relationships with patients to encourage trust, while listening to and interpreting their needs and concerns

* Ensuring the correct administration of medication and monitoring the results of treatment

* Preparing and maintaining patient records and producing care plans and risk assessments

The following list of skills and experience are desirable for this role:

* Catheters (M, F & SP).
* Injections.
* Compressions.
* Venepuncture.
* Cannulation.
* Wound Care.
* Blood Pressure.
* Leg Ulcer Management.

The list above is not exhaustive and any other skills you possess would be advantageous.
