Our client is one of the UK’s leading providers of home care and support delivering high quality care. They have over 180 branches nationwide, provide care for over 40,000 older and disabled people with 17,000 well trained and dedicated staff.

They’re now looking for a Care Coordinator to join them to cover their Kensington and Chelsea contract to assist in the day-to-day operation of the Home Care Service business, involving direct line management for a team of Home Care Workers.

Managing the weekly load of the Care Workers to ensure that services are delivered in line with contracts, you will participate in the multi-disciplinary assessment of service user needs and help to plan their care. Defining any special care needs and liaising with external organisations (CPN’s, chiropodists, GPs etc.) to develop a total package of care, you will report into the Branch Manager on a weekly basis on activity levels and outcomes for service users. As a Care Coordinator, you will engage with and positively undertake on call duties as required by the branch to ensure that a 24/7 service is provided.

Participating in staff development, training and appraisals and providing written reports of all complaints, accidents and incidents, you will keep abreast of new developments in care and conform to the Company Code of Professional Conduct in relation to the operation of the Home Care Service. Ensuring that the day-to-day Care Services are operated in accordance with Health & Safety legislation and current statutory regulation guidance on best practice.

Holding or willing to undertake an NVQ/SVQ level 3 in Care and with previous experience within a Care environment, the successful candidate will have excellent spoken and written English, good IT skills, competence in Microsoft Office and possess strong leadership skills. With excellent communication and interpersonal skills, you will be tolerant, patient and sensitive. Health & Safety / First Aid / Food Hygiene / Moving & Handling certificates would be an advantage, as would experience with Cold Harbour systems, care plan writing and accident reporting.

As this role involves travel a full clean driving licence is essential.

The hours of work will be 8:30am - 5:30pm, 40 hours in total per week with on call duties included.
