Support Worker (Hospital to Home)
Permanent - Full or part time hours available (37.5)
Hours negotiable but needing to cover North West Surrey and Surrey Downs areas
Salary starts at £16,700 per annum (pro rata) rising to £18,500 (pro rata) through performance plus mileage of 45 p per mile.

We need you to work with customers at the point of their discharge from hospital and then for up to six weeks after. No personal care will be involved, the support that you will provide will include shopping, basic help around the house, companionship and working with other agencies.
We are interested in hearing from you as we have opportunities for either full or part time work. Ideally you will be able to cover the area from Egham in the north down to Woking and then across to the border with West Sussex. Or we would consider a job share with one colleague covering the North West and one Surrey Downs. These are really rewarding roles at the heart of the community.
It’s really important to us that you have time to what you are really care about; changing clients lives for the better. So to free up your time, you’ll be given a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office as you can complete support plans on the go. You’ll also be paid between clients visit and for your mileage too.

A bit about you
We want someone like you, with the passion and motivation for helping vulnerable people make positive and lasting changes to their lives. To flourish in the role, you’ll have a practical understanding of safeguarding, risk assessments, confidentiality and support plans. You’ll be great at managing your own time, working on your own initiative, have strong communication skills and be able to positively influence people from all different backgrounds. It’s important to us that you have the compassion and empathy to work with vulnerable people in a housing related social care setting and we do need you to also have good IT skills too.
We’ll be impressed if you have knowledge of this sector, our client group and experience of working with hard to reach clients, but don’t worry as we’ll also develop your skills in this too.
We need you to have a driving licence, car and car insurance business cover as some travelling will be required.
If you are successful, we need you to have an Enhanced Disclosure Check through the Disclosure and Barring Service which we’ll pay for.
