Qualified Social Worker - Children In Need - Hounslow

Our client is a local authority looking for a Qualified Social Worker to work in Children in Need and Transition Case Work.

To be considered for this role you must have: -

• A degree in Social Work
• Hold a full HCPC registration
• Two years post graduate experience in a relevant team
• Experience of working in 0-25 Children and Young Adults with a Disability and Transition Team
• Be eligible to work in the UK
• Driving licence and business insurance would be beneficial
• DBS update is preferred by client

The role will involve: -

• To be involved in the assessment process, and reviews of support packages and child in need/support plans of disabled children and young adults and their families/carers taking into account their views, and ensure the establishment of individualised plans with particular reference to discriminated against groups. To participate in multi-disciplinary assessments, undertake key worker functions and to provide, monitor and review support services as agreed in child in need/support plans that promote the welfare of children and young adults and are in accordance with departmental policies and priorities.
• To develop specialist expertise and knowledge in working with children and young adults with a disability and their families/carers, including an understanding of relevant services and facilities which may support with children and young adults and their families/carers and where appropriate contribute and participate in the development and/or review of service provision and delivery.
• To ensure effective communication and liaison with children, young adults, parents/carers and other professionals and agencies both internal and external to the Local Authority. Ensuring that confidentiality and data protection are maintained and promoted at all times. Maintaining accurate and full records of work undertaken with service users and others, preparing reports, attending and participating in a range of meetings as appropriate to the work, e.g. case reviews, core group meetings, CIN/support plan meetings, etc and in line with Department’s policies and procedures. To participate within the duty management systems as required and supported by a qualified social worker.
• To work alongside a multi-disciplinary team, supporting a range of assessment frameworks and implementation of programmed approaches and transition to adult or other services. The ability to support parents/carers in implementing these approaches and too work directly with disabled children and young adults seeking and promoting their views within the assessment and review process.
